Case Management
for Workforce Development

To resolve information silos and achieve whole-person care, providers need a way to share patient information and make data-driven decisions. Client data needs to be secure, private, and easy to share with relevant parties. ClientTrack helps providers achieve improved care, better health outcomes, and reduced overall cost. Our highly flexible care coordination platform enables timely access to care for members of a community health neighborhood.

Platform for Food Delivery

In every community, there are people unable to meet their own dietary needs, and meeting those needs involves many moving parts, from identifying those who need the services to reporting and accountability. With so many different elements that must be managed, tracked, and followed up on, and with so much data that must be stored and retrieved for accountability purposes, it’s easy for an agency to be overwhelmed by the administrative minutiae.

Data Integration and Migration

ClientTrack brings the disparate provider elements of care management together, breaking down the silos separating different providers and enabling more comprehensive case management for vulnerable populations. By integrating industry standards with migration technologies, ClientTrack eases the sharing of patients’ health information between providers and parses legacy data into a modern database optimized for queries, reports, and data visualization.

Data Protection and Security

Eccovia’s network protection includes two-step authorization, network isolation, virtual networks, communication encryption, and site-to-site and point-to-site VPNs. All network protection begins with limiting physical access to the network environment, including physical security of Tier I assets. In health and human hervices (HHS) and anything touching on social services and personally identifiable information (PII), data privacy and security is always a top concern.

Document Management and Storage

Document Management and Storage (DMS) is among the most important features a case management system can provide. DMS allows organizations to capture, track, store, and share documents (e.g. PDFs, word processing files, digital images) connected to their clients. Files are stored in Azure Blob storage, which is designed with geo-replication and flexibility to scale as needed. Data is stored with advanced threat protection and is encrypted at rest.

Comparable Database for Domestic Violence

With ClientTrack’s intuitive intake workflows designed to capture data required for standard HUD reports like CAPER and APR, combined with easy-to-use reporting tools, compliance and accountability are easier than ever. ClientTrack helps you free up time from data input and focus on what matters most—helping people recover and heal. ClientTrack can serve as a resource for housing management, case management, closed-loop service referrals, and more.

Our team is headed by industry experts with many decades of collective experience in HMIS and HUD requirements, supporting continuums of care (CoCs) from rural, urban, and statewide implementations.

Refugee Resettlement

Providing life-changing refugee services is challenging. It involves immense amounts of data to provide effective case management, which tracks services and helps refugees begin new lives, processing that data into required reports for the Office of Refugee Resettlement (ORR). ClientTrack connects local and statewide refugee-service providers, empowering them to help refugees find a better path to self-sufficiency through client-centered case management.

At the core of Eccovia’s data management philosophy is the belief that data is not just information, but an extension of the individual.
