Leading Medicaid, Medicare, and Duals health plans are embracing Whole Person Care — the coordination of physical, behavioral and Social Determinants of Health – by harnessing data and technology to identify vulnerable populations and coordinate care real time to achieve improved outcomes.
In post-pandemic times, it is more critical than ever that health plans meet members where they are with targeted, integrated programs that influence member behavior, boost outcomes, and control costs. This conference will explore how industry thought leaders are developing a whole person care infrastructure from the ground up, based on quality data and actionable insights. It will examine how to build and expand a true Whole Person Care foundation, and provide coordinated care that addresses the physical, behavioral and social needs of complex members. Leading plans will share how they are identifying those most at risk, working toward eliminating barriers to care, improving access, and addressing SDoH and health equity as a focal point of their strategy.
