Closing the Loop: How Atlanta Handles Coordinated Entry

Case management technology is crucial, but it only works when a community has a shared data-governance model for mitigating homelessness. In this special webinar, Atlanta’s CoC lead agency, Partners for HOME, will join Eccovia and the Institute for Community Alliances (ICA) to help communities learn how to:

  • Achieve closed-loop referral for coordinated entry via access, assessment, prioritization, and referral;
  • Attain a shared data-governance agreement among all community organizations, for better data quality and privacy all around;
  • Utilize technology to solve data-sharing problems with coordinated care; and,
  • Develop community partnerships that multiply social service efforts more effectively.
