Next week in Phoenix (September 18-21, 2016) federal, state, and local human service leaders will be coming together at the APHSA ISM 2016 conference to discuss key trends relating to technology for health and human services. Whether attendees come from large, regional Medicaid Accountable Care Organizations (ACOs) or smaller, community-based organizations which work directly with the ACOs, the challenge is the same — to effectively coordinate care across multiple providers for individuals and their communities.
Care coordination involves organizing patient care activities and sharing information across all concerned agencies and providers to achieve safer, more effective care. Well-designed, targeted care coordination can improve outcomes and reduce costs for everyone: individuals, providers, and payers. One community organization leading the way in case coordination is Childhelp Children’s Advocacy Center of Arizona.
Since 1998, Childhelp Arizona has served abused and neglected children by providing treatment, intervention, and investigation services. The goal of the center is to provide an individual care plan for each child which addresses their complete well-being. These plans include medical treatment, mental health therapy, investigational resources, and victim support services to address immediate safety concerns. Childhelp Arizona collaborates with community partners such as the Phoenix Police Department, the Office of Child Welfare Investigations, Phoenix Children’s Hospital, Maricopa County Attorney’s Office, and the Arizona Attorney General’s Office. With a caring focus, the partnership is a highly successful example of multi-agency collaboration to coordinate whole-person care for children and families in the community.
On Monday evening at APHSA ISM 2016 attendees will be invited to help stuff 2,500 backpacks with needed supplies for Childhelp Arizona.
